Information clause regarding the use of image
Pursuant to Article 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (EU Official Journal L 119, p. 1) - hereinafter referred to as "GDPR" - we inform that: 1. The Controller of your personal data (hereinafter referred to as the "Controller") is Podlaska Wytwórnia Wódek "POLMOS" Joint Stock Company (hereinafter referred to as "Polmos"), with its registered office at Frascati Street 12, 08-110 Siedlce, entered into the National Court Register under KRS number 0000027198, Tax Identification Number (NIP) 821-00-07-656. 2. Personal data will be processed for the purpose of using your image in materials published or authorized by Polmos in connection with your participation in the event celebrating the 30th anniversary of the Chopin brand, on the website and social media platforms of our organization, for promotional purposes, based on Article 6(1)(a) of the GDPR. 3. The provision of personal data is voluntary, but necessary to achieve the above-mentioned purposes. 4. Personal data will be stored for a period compliant with legal regulations or until the withdrawal of consent. 5. Recipients of your personal data will be: - Entities authorized to obtain personal data under the provisions of the law, - Persons authorized by the Controller to process data as part of their official duties, - Entities to whom the Controller entrusts the performance of tasks that require data processing (data processors). 6. You have the right to request access to your personal data, rectification, erasure, or restriction of processing, the right to object to the processing, the right to data portability, and the right to withdraw consent at any time. 7. You have the right to lodge a complaint with the supervisory authority - the President of the Office for Personal Data Protection.
Consent to the use of image
I hereby declare that, in connection with my registration for the Blue Moon event, I give consent to the free recording in the form of photos and videos, and the use of my image by Podlaska Wytwórnia Wódek "Polmos" S.A. (the Company).The consent includes recording, editing, reproduction, and use of the photos and video materials through any media and in the Company's marketing materials in accordance with applicable law. I also waive rights related to controlling and approving each use of photographs featuring my image. Legal basis: Article 81(1) sentence 1 of the Act of 4 February 1994 on Copyright and Related Rights.
